Fees and payment information

We work with most insurance providers, and in most cases we are able to provide you with an estimate at the time of your first visit. If you need to make any financial arrangements, please let us know. Charges for office visits and electrodiagnostic tests are determined by complexity and length of time spent, and vary with the type of test. If you have any query's regarding which insurance plans we accept or any patient billing concerns, please call us. Questions regarding your coverage and benefits should be directed to your employer or insurance company.

If another doctor has referred you to us, please let us know, so that we can report to him or her promptly. If you belong to an HMO, you need to bring your referral to your visit.

What to bring to your first appointment

• Your insurance card
• Physician referral forms if required by insurance
• A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
